It's the execution that's the issue. They made it super awkward which is why everyone is mentioning it. It comes across as performative.
Then you have the deadname encounter in Last of Us Part 2 where enemies will refer to your companions deadbame repeatedly in a mocking tone. After the encounter Abby simply asks, "Do you want to talk about it?" To which Lev responds "No". They leave it at that and continue on.
So it's the execution folks are issue with.
